REDUCING HOSPITAL RE-ADMISSIONS FOR OLDER PEOPLE Happy to Help is working with local services and hospitals to offer a subsidised 6 week home support packages to isolated older and disabled residents returning home from hospital. The programme will complement and enhance personal care packages that form care plans for patients and will support them to manage the home, shop, collect pensions and pay bills while they recover at home. This reduces the emotional burden of not being able to manage the home, prevents isolation and loneliness whilst reducing the proven increased risks of readmission
